Adoption application

The application form does not obligate you to an adoption but provides us with much needed information about yourself and the dog you are looking for so we can help pair you with the perfect companion now, or in the future. All Safe Haven dogs are fully health checked on a regular basis, and are placed spayed & neutered, micro-chipped and up-to-date on vaccinations. Adult dogs are $100 / puppies require a $150 deposit.
